Service Description
Lifestyle IV provides mobile IV hydration services, including but not limited to hydration therapy, vitamin infusions, and electrolyte replacements, at the convenience of your location. Services are provided by licensed medical professionals in accordance with applicable laws and regulations.
Client Eligibility
To use our services, you must be at least 18 years of age, or have a legal guardian's consent. You must disclose any medical conditions, allergies, or medications you are currently taking that may impact the safety or effectiveness of the treatment. Failure to disclose such information may result in the refusal of service.
Booking and Scheduling
To book an appointment, you must provide accurate and complete information, including your full name, address, contact information, and payment details. Our team will confirm your appointment based on availability. We reserve the right to refuse service to anyone at our discretion.
Payment Terms
By booking our services, you agree to pay the full amount for the service rendered. Payment must be made prior to or at the time of service unless otherwise agreed upon.
Cancellations and Rescheduling
You may cancel or reschedule your appointment up to 4 hours in advance without penalty. Cancellations or reschedules made within 4 hours or less of the scheduled appointment may incur a cancellation fee of 20% of the price of the treatment.Medical

Liability
